UT Tyler COVID-19 Report Form

Overview

This report is intended to provide a mechanism for members of The University of Texas at Tyler (UT Tyler) community to self-report positive Coronavirus 2019 (COVID-19) diagnosis, suspected positive diagnosis, and self-quarantine requirements because of direct exposure or presumed exposure. This information is given voluntarily and is only shared with the Office of the Dean of Students (for student reports), Office of Human Resources (for employee reports), and the Incident Response Team. If you need to speak with someone immediately, please contact UT Tyler’s COVID-19 Hotline at (903) 565-5999.

Student Information

If you are a student, please note an automatically generated email will be sent to you if you provided your email address in the Background Information section. You will then be able to provide this letter as an interim absent notice to your faculty until a Case Manager from the Campus Assessment, Response, and Evaluation (CARE) Team follows up with you. Furthermore, updated student-specific information is available on the Student-Related Information Concerning COVID-19 page.
